I’ve read scientific papers in University that reviewed patient safety and more specifically anesthesiologist errors.
It’s surprising how many errors occur – but the underlying reason most often is latent errors that happen to occur at the “sharp” end (those administering tablets, monitoring, operating etc).
The book outlines the classic “swiss cheese” develop and reminds us we are quick to blame the person that committed the error but, we need to look closer at the systemic or latent errors.
I’ve everlastingly had an interest in pursuing more human factors work in regard to patient safety and this was a fantastic resource to remind me of various medical error sources.
I also urge James Reasons “Human Error” for an indepth review of Human Error – slips, lapses, mistakes etc.

As the science of Patient Safety continues to grow and we are searching for the “manual” to patient safety principles and examples, I am delighted to learn this new book, Understanding Patient Safety by Robert Wachter.
Dr. Wachter has a vast understanding of the topic of patient safety and he speaks openly and honestly about those things within the medical arena that we need to fix to make the healthcare system safer for all of us. His writing style is simple to know and he uses brilliant examples to illustrate the key principles and keep the book fascinating.
Similar to his writing in Internal Bleeding, he uses real life scenarios to illustrate how medical errors and near misses occur, without placing blame on those individuals caught up. He focuses on the kaput systems and encourages us all to find a way to fix the systems so that we can provide better, safer healthcare.
As a professional working in the field of Patient Safety, I highly urge this book for everyone who works in the healthcare field and for anyone interested in understanding and protecting their loved ones from errors that might occur when using the healthcare system. I have even given a copy to my Mother!

Wachter has covered a complex topic with fantastic clarity and fine style. The book is organised logically to allow the reader to follow from start to end or “drop in” on a topic of interest. The interliniking of chapters and thoughts is well constructed through example and thwart reference – again aiding a rapid uptake of the knowledge sought.
The careful examination of language that starts the book is very helpful to beginners but takes some concentration to know. This section is helpfully supplemented by a glossary that probably should also be read early.
The examples used throughout the book are both elightening and frightening! Use of examples assists the reader to know the complexity of safety issues whilst focussing on the current topic.
The writing style is clear and simple to know. A minimum of medical language is needed and many specialist terms are clarified by the side of the way. I feel confident beginners and experienced staff will find this acceptable.
Wachter starts with definitions and the overall situation, leads us through common causes and issues and ends with recommendations for action.
Each chapter ends with a summary of key points that is a helpful adjunct to the main text.
International readers will also benefit from Wachters detailed coverage of the issues and examples from within the US as he manages to keep the discussion largely universal in its application.
Overall, I have found the book very helpful on getting my specialist area of interest, Clinical Engineering, and its role in Patient Safety into context. Highly not compulsory reading and for this reviewer an instant favourite.

If “Internal Bleeding” was an inspiring book; this is a comprehensive, modern, didactic, updated and most of all, very helpful “text book”.
Simple to read, with graphic examples, integrative tables and graphics, take home key points and smart design; this book attest Wachter’s unique writing talent.
Seeking patient safety should become a proven clinical skill for every health care worker and, “Understanding Patient Safety” is an brilliant starting top. Also anyone who is or will become a patient would profit with it’s reading.
So helpful for culture exchange, I wish there’s soon a spanish version available.
